Practice Transformation Institute’s May 11th Symposium on the Patient-Centered Medical Home Brings State and National Leaders Together with Goal of Putting the Pieces Together

Troy, Mich.---April 22, 2010 --- Practice Transformation Institute (PTI), Michigan’s leading provider of CME and IACET-accredited experiential learning programs for the Patient-Centered Medical Home and other primary care transformative initiatives, announces it will host a one-day symposium on the Patient–Centered Medical Home at Macomb Community College on Tuesday, May 11. Entitled, Putting the Pieces Together, the event features some of the most prominent proponents of the Patient-Centered Medical Home movement and is designed to bring participants up-to-date on P-CMH initiatives at the regional, state and national level.

The Patient-Centered Medical Home is a national health care movement that establishes the primary care physician’s office as the patient’s medical home, with the personal physician coordinating all of a patient’s medical care to improve patient health and decrease costs. The Patient-Centered Medical Home (P-CMH) is a concept, not a place, that puts the focus back on the patient and treats the whole person.

PTI also hosted Michigan’s first comprehensive symposium on the Patient-Centered Medical Home in September 2008, well before the movement had any traction in Michigan.

"The health care community is a huge machine and it can take years before a legitimate change in patient care delivery can really take hold," explains PTI founder Ewa Matuszewski, who also serves as CEO of physician services group Medical Network One. "The 2008 symposium was an eye opener for attendees who were just learning about the P-CMH and it served as a catalyst for the primary care community to get aggressive in making medical homes a reality in Michigan. Now, our state is actually among the nation's leaders in incorporating Patient-Centered Medical Homes into our health care system, both at the practice level and from the perspective of payer support."

The May 11th symposium speakers are an elite group of influential regional and national leaders in the patient-centered medical home and include:

Ms. Matuszewski continues to play a key role in representing Michigan at the national level on P-CMH issues being addressed in Washington, D.C. and is an active participant of the Patient-Centered Primary Care Collaborative (www.pcpcc.net), an organization that originated and continues to spearhead much of the P-CMH initiatives.

The Practice Transformation Institute is hosting the symposium in cooperation with nearly 20 health care partners and employer groups. The objectives of the Symposium are to outline and reaffirm the principles of the Patient-Centered Medical Home; recognize the impact of multi-payer demonstration projects in various states; explain the advantages and identify action plans for the purchase of Patient-Centered Medical Home care, an area of particular interest to employer groups; describe population-based outcomes and adherence programs for self-care management programs, and; demonstrate the impact of the P-CMH on public policy, payment and provider reimbursement systems. A series of breakout sessions will be offered in the afternoon. The event is geared to physicians, health plan administrators, health care executives, health educators, employer groups, elected officials, medical school and university administrators, and progressive healthcare advocates.
